What are Biostimulators?

Biostimulators are a class of injectable aesthetic treatments that function by triggering the body’s fibroblast activity — stimulating the production of new collagen and elastin rather than simply adding volume through gel displacement. The two most established biostimulators in clinical use are Sculptra® (poly-L-lactic acid) and Radiesse® (calcium hydroxylapatite).

Sculptra® (Poly-L-Lactic Acid / PLLA) Sculptra® is an FDA-approved injectable made from poly-L-lactic acid — a biocompatible, biodegradable synthetic polymer that has been used in dissolvable sutures for decades. When injected into the deep dermis and subcutaneous tissue, PLLA microparticles trigger a controlled inflammatory response that stimulates fibroblast proliferation and new collagen synthesis. The PLLA is gradually absorbed over time; the collagen it generates remains. Results develop slowly over 3–6 months and can last 2 years or more.

Radiesse® (Calcium Hydroxylapatite / CaHA) Radiesse® is an FDA-approved injectable composed of calcium hydroxylapatite microspheres suspended in a water-based gel carrier. CaHA is a mineral compound naturally found in bone and teeth — making it highly biocompatible. Upon injection, the gel carrier provides immediate volume while the CaHA microspheres act as a scaffold, stimulating collagen and elastin production as they are gradually resorbed. Results are both immediate and progressive, typically lasting 12–18 months or longer.

Both products are non-hyaluronic acid, non-reversible with hyaluronidase, and require a thorough assessment and conservative approach to treatment planning.
